Vragenlijst voor het meten van kwaliteit van leven. Het instrument meet, zoals de naam al zegt, 5 dimensies (mobiliteit, zelfzorg, dagelijkse activiteiten, pijn/andere klachten, angst/depressie). Elke dimensie bestaat uit één item waarbij drie niveaus worden onderscheiden (geen problemen, enige problemen, veel problemen).

Variant van de SF 20 en SF-36 (zie aldaar), een vragenlijst voor het meten van kwaliteit van leven. De SF-24 bestaat uit 7 dimensies: fysiek functioneren, rolbeperkingen, pijn, algemene gezondheidsbeleving, vitaliteit, sociaal functioneren, en geestelijke gezondheid.

Medical Outcomes Study 36-Item Short Form Health Survey
Vragenlijst voor het meten van kwaliteit van leven. Het instrument bestaat uit 8 dimensies: fysiek functioneren, rolbeperkingen door fysieke gezondheidsproblemen, pijn, algemene gezondheidsbeleving, vitaliteit, sociaal functioneren, rolbeperkingen door emotionele problemen, en geestelijke gezondheid. Deze 8 dimensies kunnen bovendien gesommeerd worden in een lichamelijke en een psychische hoofddimensie
